If you live with a chronic condition, the start of a new calendar year means your GP Chronic Condition Management Plan (GPCCMP) resets, giving you access to up to 5 Medicare rebated allied health visits again.
What is a GPCCMP?
A GPCCMP is a plan created by your GP and nurse to help manage a long term or ongoing condition. It looks at your overall health needs and supports coordinated care between your GP and allied health providers.What conditions can a GPCCMP help with?
This can include (but is not limited to):
• diabetes
• endometriosis
• ADHD
• asthma
• arthritis
• chronic pain
• osteoporosis
• mental health conditions
• cardiovascular conditions
• and many other ongoing conditions
What does a GPCCMP give you access to?
✔️ Up to 5 Medicare rebated allied health visits per calendar year
✔️ Care from providers such as physiotherapists, dietitians, podiatrists, exercise physiologists and more
✔️ A coordinated care plan tailored to your health goals
✔️ Ongoing three monthly bulk billed GP reviews to keep your care on track
If you already had a plan last year, your rebates reset from 1 January.
