All of the GPs here are trained in the management of chronic (long term) health conditions. These conditions include, but are not limited to, high blood pressure, diabetes, high cholesterol, asthma, emphysema, arthritis, osteoporosis, long term back pain and many others. They offer Chronic Disease Management Plans for existing and eligible patients.

At Next Practice Prahran, all of our GPs are experienced in managing chronic (long term) health conditions, including but not limited to high blood pressure, diabetes, high cholesterol, asthma, emphysema, arthritis, osteoporosis, and ongoing back pain. We offer Chronic Disease Management Plans for existing and eligible patients, designed to support your health over time and connect you with the care you need.

Medicare Funded Chronic Disease Care Plan (GPCCMP)

What is a care plan?

A chronic disease care plan is a personalised health plan developed by your GP and nurse to help manage ongoing health conditions such as diabetes, endometriosis, adhd, asthma, heart disease, arthritis, and many more.
Your care plan focuses on your individual health goals and provides a structured approach to managing your condition, with support from a broader healthcare team where needed.

Who is eligible?

You may be eligible for a care plan if you have a health condition that has lasted, or is expected to last, six months or more.

Your GP will assess your eligibility and work with you to create a plan that is tailored to your needs.

What do you get with a care plan?

  • A personalised and supportive plan to help manage your health
  • Access to up to five Medicare subsidised visits per year to allied health professionals, including physiotherapy, dietetics, podiatry, psychology, and more
  • Regular care plan review appointments to help keep your health on track

What to expect at your appointment

Your care plan appointment is designed to feel supportive, structured, and collaborative.

  • You will first meet with one of our nurses, who will spend time understanding your health history and helping you set achievable goals
  • Together, you will explore whether involving allied health providers would be helpful as part of your care
  • Your plan will be documented and shared with you and your healthcare team
  • You will then have a brief follow up with your GP to review and confirm your plan

Our nursing team will continue to support you with your care plan, with follow ups across the year and a GP review typically completed within 3 to 6 months.

Cost and Medicare information

At Next Practice Prahran, we recognise the importance of accessible chronic disease care. We aim to bulk bill care plans for Medicare card holders wherever possible.

*Please note that appointments with Dr Nisha Iyer are generally privately billed, with bulk billing at her discretion.
